This article provides a step-by-step on how to complete the online application form. To view 'Step 1/3: How to Start a Police Check Application' click here. Once you have created your account, you will need to complete the online application form. To complete your application, you will need to provide the following information: Step 1. Input Your Current and Previous Names You will need to enter your full name, as well as any previous names or aliases. To add a previous, maiden or alias name, scroll to the 'have you been known by any other names' field and select 'yes'. An additional field will appear below, where you can enter your additional name(s). 'Single Name' means that you are known by one name only and do not have a combination of a Surname and Given name. Do not tick the 'Single Name' box unless your name is a single name Step 2. Input Your Birth Details You will need to input your date of birth, gender, and place of birth, including the suburb and town if you were born in Australia. Click 'continue'. Step 3. Input Your Current and Previous Addresses You will need to input your: Current and previous addresses (for the last five years) To add a previous address, click the 'add additional address' button You do not have to provide exact dates for your address. A rough estimation will suffice Step 4. Input Your Contact Details Once you have entered your address you will need to enter in a contact number. Below the 'contact number' section there will also be an option to set a police check renewal reminder. This step is optional. You do not have to select any reminders. Once you have completed all relevant fields, click 'continue'. Step 5. Input Your Licence Number(s) (optional) If applicable, you will need to enter your licence number Click 'continue' Step 6. Input Your Purpose of check Please review the purpose fields you completed at the start of the application process If you wish to change any of this information, you can edit this information here You will also need to provide the State/Territory and the Location, Town or City where you’ll be working If you will be working across states/territories or across locations, please provide the location of the company headquarters If you will be working overseas, in the State/Territory field please select ‘Other’, in the Location, Town or City please input the name of the country If you have been requested to provide a police check to a recruiter, you can provide the recruitment companies name in place of the employer name When providing the reason for your check, please note the following: Acronyms and abbreviations will not be accepted There is a 100-character limit for each of the fields Multiple check purposes will not be accepted For further information on how to complete the check purpose, please see here Once completed, click Continue Step 7. Authorisation to Disclose If you would like the result of your police check to be forwarded to a third party directly (e.g. your employer). You will need to: Select Yes Input the organisation/employer's name, ABN and email address If you do not wish for InterCheck to forward your certificate on to a third party, click No Click Continue Step 8. Review Once you have completed the application form, please review all of the information provided and make sure its all correct. If you wish to update any information, click Edit. Once all the information is correct and ready to submit: Read the agree to the terms and conditions by clicking 'by checking this box..' A ‘Digital Consent’ ****pop-up will appear, click Yes Click 'I understand that I need to provide required documents to finalise my application' Click Confirm After clicking Confirm you will proceed onto the ‘Online IDs’ section of the application form.
