Where to Locate Under 18 Consent Form

To locate the consent form please log into your Intercheck account. 

From there you can locate your consent form on the ‘Online IDs’ page. Under the heading ‘consent form’.

Click on the link 'consent form' to download the form and print or sign digitally. Once you have signed the form, please upload it under the 'Consent form section'.
