Where to Locate Under 18 Consent Form

When completing your application, if you are under 18 you will be prompted to download your consent form on the 'Online IDs' page. You will need to:

• Click 'Download Consent'
• Have the consent form signed by both yourself and your parent or legal guardian

• The National Police Checking Service also requires that we verify your parent or legal guardian's identity and establish their link to you. 
• Please have your parent or legal guardian provide each of the following documents:
  • one of the documents listed as a 'Commencement of Identity Document' (such as parent/guardian's Australian Passport)
  • one of the documents listed as a 'Primary Use in Community Document' (such as parent/guardian's driver's license)
  • one of the documents listed as a 'Secondary Use in Community Document' (such as parent/guardian's Medicare card)
  • a documentary link between yourself and your parent or legal guardian (such as your birth certificate or court order)
